Forms

For all employees who have access to use our Accommodate online portal, we request that all accommodation requests and medical documentation are submitted online. 

If you are unable to utilize the online portal, the PDF files for forms that our office accepts can be found below.  You may download the needed form and submit completed forms by email to dasemployee@emory.edu or by fax to 404-727-1126.

Workplace Accommodations

Employees must complete and return the request form and provide sufficient medical documentation (either the Medical Inquiry Form or documentation that meets the requirements based on type of disability)
