Supporting Documentation and Forms
If you have a visible or readily apparent disability, documentation is not required to initiate a request for ADA accommodations. If your disability is invisible, documentation is required. However, acceptable documentation varies based on:
- The type of disability/condition
- The duration of the impairment
- The urgency of the need for accommodation
- The employee’s access to medical documentation
The following documents may be sufficient to initiate a request for accommodation:
- Medical visit summary or hospital discharge summary that discloses the associated diagnosis and provides a summary of the individual’s current health status.
- Screenshot of diagnosis list - view must clearly show the person is logged into their own patient medical record.
- Letter from a licensed healthcare provider that discloses diagnosis, current level of functioning, extent of impairments, and recommendations.
- A completed DAS Medical Inquiry Form or Parking Accommodation Medical Form
