Returning Employee Accommodation Request
If you have already previously submitted Employee Accommodation Request Form, you will not be able to submit another form using the public request form link.
Employees who meet the following criteria can use this link to access their DAS record for the following purposes:
- I have submitted an Employee Accommodation Request Form but need to submit supporting documentation for the first time to continue in the interactive process.
- I previously registered with DAS for a parking accommodation and would like to request a workplace accommodation.
- I previously registered with DAS for a workplace accommodation and would like to request a parking accommodation or a different workplace accommodation.
- My accommodation has expired and I would like to request an extension by submitting updated supporting documentation.
- I would like to review my DAS record to see what documentation has been uploaded or to view a copy of accommodation agreements.
